There is a $250.00 wait list fee required before being placed on the wait list. Once the fee is collected (check made out to MSC) and the application and this letter are submitted to the office, you will then be placed on the wait list. Once your name surfaces to the top of the list and a membership becomes available you will be called to either except or decline the membership.

Accepting the membership: If you decide to accept the membership, you will need to make sure you have filled out all the required forms. This includes the payment plan option form, application, insurance wavier, family information form. Make sure to sign and date all the paper work. Submit the forms along with the payment plan you choose, making sure to subtract the $250.00 (wait list fee) to be applied toward the membership.

Declining the membership: If you decline the membership for any reason, or ask to be taken off the waitlist for any reason you will forfeit the $250.00 wait list fee.
